Client Service Agreement for Professional Coaching Services

This Agreement is made and entered into as of date agreed upon  (the "Effective Date") by and between:

(“Coach”)

and

You, the client.

(“Client”)

1. Services Provided 

The Coach agrees to provide professional coaching services to the Client, which may include but are not limited to personal development, career coaching, goal setting, and accountability.

1. Session Details 

- Frequency: Coaching sessions will occur based on the client's desired frequency and Coach’s availability, for a duration of [60/90 minutes per session], or may be customized based on the type of coaching session/package/membership purchased.

- Location: Sessions will be conducted via video conferencing platform.

- Duration: The coaching relationship will begin on 9/19/2024 and will continue until services purchased have been conducted and/or discontinued, or unless terminated earlier in accordance with this Agreement.

1. Fees and Payment Terms 

- Fee: The Client agrees to pay the Coach a fee of $250 per session, or $1000 for a package of 5 sessions. 

- Payment Method: Payments can be made via credit card, PayPal, Venmo or Zelle, and are due prior to the first session.

1. Cancellation Policy 

- The Client must provide at least [24 hours] notice to cancel or reschedule a session. Cancellations made within 24 hours of the scheduled coaching session will not be refunded, and may or may not be rescheduled based on the availability of the Coach.

1. Confidentiality 

The Coach agrees to maintain the confidentiality of all Client information shared during coaching sessions, except as required by law or with the Client's consent.

1. Responsibilities of the Client 

The Client agrees to:

- Attend scheduled sessions on time.

- Be open and honest during coaching sessions.

- Take responsibility for their own progress and decisions.

- Complete requested in-between session assignments & goals-aligned exercises 

1. Limitation of Liability 

The Coach is not liable for any decisions made or actions taken by the Client based on the coaching received. The Client understands that coaching is not a substitute for formal professional mental health services.

1. 3 Termination of Agreement 

Either party may terminate this Agreement with written notice to the other party. Any payments made for future sessions will not be refunded for unused sessions, should Client request to end services prior to use of forementioned unused sessions.

1. Governing Law 

This Agreement shall be governed by and construed in accordance with the laws of the State of Illinois.

1. Entire Agreement 

This Agreement constitutes the entire understanding between the parties and supersedes all prior agreements, whether written or oral.
